Clinical Nurse Case Coordinator

The Clinical Nurse Case Manager (CNCM) is responsible for coordinating patient care to ensure optimal healthcare outcomes and resource utilization. Working collaboratively with physicians, nurses, other healthcare professionals, and volunteers, the CNCM evaluates patient needs, develops individualized care plans, monitors progress, and facilitates access to appropriate resources and services. The CNCM is also responsible for reviewing and evaluating clinical data, including laboratory, imaging, and medical records ordered by providers, to inform care decisions.
